it comes up with instructions. I looked at a couple of them and I'm sure it's what you're looking for.
Otherwise, I know I've seen the ones that just slip over the head without having to velcro or tie them. For those, I would think it is only a matter of cutting a hole near one end for the baby's head to fit thru and sewing a ribbing (stretchy like on t-shirts) around the hole.
If your niece does use velcro -- which by the way is very nice to easy because it is easier to put on/take off than trying to tie strings-- Make sure to tell her when she washes them to have the velcro attached to each other. Otherwise it snags on everything!!! I learned this from experience.
